For the most part, you pick the next one up (+2"). I usually get size 32 dress pants and size 34 belts.

Well, j's recommendation is the one usually made. However, I don't follow it. You want the belt to be cinched to the right tension with the prong in the center hole of the five. I've found that this is more often the case with belts marked with a length that corresponds to my waist measurement. Thus, if you're a 34" waist, this would mean a 34 belt.

j: Just a small point. Your signature quotation from Freud was a statement he made with maximum sarcasm intended. I'm sure you know this, but I'd hate forumers to mistakenly think that an iconic figure in the history of psychology/psychiatry was a Nazi sympathizer!

Really? I find that this is the case with Brooks Brothers, which seems to cut everything large to appease their "growing" clientele, but otherwise the 2" larger rule works for me.

Yeah, I have some size 32 belts that fit, and some 36s, but mostly a 34 is my size. Once you factor in the stretching/alteration of size 32 pants (I'm more like a 32.5 or so) plus tucking in a shirt and maybe an undershirt it ends up being more like 34".
