I have notice that some of my shoes and boots show what I would describe as "asymmetric" creasing on the tops: in general, the left shoe shows more creasing that the right. I don't mind creasing, it's the asymmetry that bugs me, as in my (possibly ignorant) eyes suggests poor quality shoes.
The quality of the leather should not be a major problem (I would hope). My shoes are not Vass, but most of them are in the 250-350 dollar range (Ferragamo, Bruno Magli, Testoni, Grenson, Barker, Magnanni etc). I can post pictures, although my shoes are nothing to brag about.
I like my shoes a little on the roomier side (for example, if a 9.5 feels a little tight and a 10 a little roomy, I would likely go with the latter)--but definitely not large on my feet. It could be that my right foot takes up more "volume" that the left--it feels like there is less space between the top surface of my right foot (over the metatarsal bones/metatarso-phalangeal joints, for those in the know) and the shoe vamp.
I have tried insoles, but shoes feel too tight with those. It might be that the heel size is okay, and I simply have "slender" feet at the front section. Are there insole "fronts"? If so, how practical are they? Any other ideas?
