Boss: yes, but the Chairman (NOT KAGA) - everyone calls him Mr. ZZZZZZ. Even his family. I call him Mr. ZZZZZZ, or Sir for short. He calls me Sir as well, so it evens out.

My boss' name is Edward, I call him Ted, Teddy if I'm being a wiseass to him. I work at a pretty laid back accountant firm I guess haha.

I always addressed the boss by their first name and I have told people who work for me to call me by my first name.

Had a guy in a previous job that I became good mates with, went out for drinks, had the occasional spliff, and only towards the end of my tenure did I find out that in fact he had a PhD. Though if I was to ever call him Dr xxx he'd probably laugh at me and tell me to shut up.

First name basis with everyone at every job I've had. Don't think that is a problem really, though I must admit at times I feel akward about doing it with certain people.
