They are not a half-size too small. I have high-instep and my feet are wide but relatively short. Going a size up would engender a huge heel slip. I have to pick my poison. I am fine with the otherwise dreadful V-shape from the loose lacing. Over time, I should be able to tighten them. Even if some V-shape remains, I am fine with that. I am not going bespoke, after all.

I'm curious if you are intentionally leaving them laced loosely during the break-in period, so that they don't end too full in the instep? Because if not, I'm wondering if maybe those aren't just a half-size too small for you, given the other issues with the right heel and the toecap and so on? I'm curious whether you would size up if you had to do it again. (And I forgot if you mentioned how you sized these relative to other brands, but would be appreciative of a reminder.)

I hope they continue to feel 'more better' every time. They look fantastic, and I am supremely jealous. :)

I own two pairs of Meccariello and I am starting to understand the extremely high reputation of the Maestro. His work is impeccable.

Another comment about the breaking process. When I got this pair, it was almost impossible to keep my right foot inside it for over 30 minutes; my instep felt strangled. Over time, things shifted enough or expanded to accommodate my high-instep wide right foot. It is still a bit uncomfortable after many hours but it is getting better every time.

Nope, no way to do that in China. Width would have been welcome but not length.

oh your size is already at upper range? How big of a size run does Medallion have for their AM?

edit: I guess you just mean wider width were not available as RTW at Medallion

Medallion does not carry "large" sizes, which means anything from UK10 up. I am a measly 10.5UK but there was only one pair there, from TLB Mallorca, when I visited. I am not a boots guy anyway, so the chukka being a bit uncomfortable is not an issue. The other pair has been ordered in the correct size (10.5UK regular for the left foot, wide for the right).

This was part of an order with 3 pairs. First one that has been worn.

It's a Decurion 3 in Chisel 2. It's 11UK (I'm a 12 US) and I think the fit is great.
