a fitting question

I am interested in a Brunello Cucinelli cotton blazer listed in my size (42), but since it will be an online purchase, I cannot try it on first.  Here's my dilemma: my shoulder measurement is 20," but the jacket's shoulder measurement is 18.5."  Will this work for me?

John, depending on the cut of the shoulder, I can go between about 19" (not ideal, but it works) and 18.25" (if the shoulder is cut less sloped). Anywhere less than 18.25 and it gets really dicey for me -- err, doesn't fit right. An 18.5 is like a 39R in Zegna, I think -- and you probably know how that would fit.
