Do they just shorten the zipper?

Not possible, afaik. They'd either want to shorten the sleeves from the shoulders or recut the area around the zippers to re-position them while shortening from the cuff. Both ways are a lot of work.

kj- i think they just cut the zipper and attach a new end-piece but i'm not really sure.

I think they would do it at the shoulder, but I'm no expert. I think that's a relatively common procedure, although obviously more expensive than a normal cuff job.
