In Alexandria, I have been pleased with alterations (basic stuff: cuffs and waist on trousers, sleeves on jacket) done by Elegant Rochee's on Duke Street in the Landmark area. They also do MTM (and perhaps bespoke) suits and shirts, so I would think their in-house tailor could do button holes as well.
In DC, I've had good success with Joe Sauro for basic alterations.
I've also had work done by Christopher Kim in DC. Does good work, but quite pricey.
Years ago, I had Georges de Paris do some alterations to a suit. Work was good but service was poor. I expect that is because his focus is on his bespoke work. (Plus, he's a nutter.)
Off topic: Anyone else notice how many tailors have photos with presidents for whom they have apparently made suits? Sauro and de Paris in particular come to mind. One gets the impression that every new President is handed a list of tailors whom he must appease.
