Yep. Often combined with dehydration and/or a significant size disparity between the fighters. See, e.g., Gerard McClellan, Duk Koo Kim.

It's not impossible. I suppose one caused internal bleeding and wasn't treated he could die. However, that would have to be one massive blow to the solar plexus. Most organs are behind the rib cage which can take multiple hard blows. (Yeah I said organ and blows in the same sentence.) The liver is partially behind the rib cage so it has some protection. Ever hear of the "Mexican liver punch?" That is a shot on the unprotected part of the liver, resulting in toxin release into the body. Very unpleasant and will put you in the hospital for a couple days. I don't know if it can kill you though.
