Sleek lace-up boots in Bill Blass ad

Can anyone identify who made (makes?) these boots, or boots like them, and where to get them? These boots were featured in 2 full-page Bill Blass photo ads in the March 2003 GQ.  The ads showed how a Bill Blass suit could be worn with the suit jacket and tie, or with a sweater, and the same boots could be worn for either the dressy or casual outfit. These classy boots are shiny black, sleek, with contrasting stitching along the eyelets, use hooks in place of the top 3 or 4 eyelets, and a tab along the back of the boot. Sorry the scans aren't great.

That looks like a Stacy Adams boot to me.

If you go much further back in the GQ morgue, you might dig out the article from maybe 1996 or so about "the boot shoe". I distinctly remember this article having several pictures of various makers' "boot-shoes". I don't remember any of those makers' names, though.

I knew I had seen something similar... http://www.amesbury.co.uk/bespoke/en/group_4_30.html Has anyone actually tried Amesbury as an alternative to Lobb bespoke? I would love to hear about the results.
