I have the same shirt in both an 18 and a 16. Â The sleeve lengths measure to about 35.75" for the 16 and 36.5" for the 18. Â I got this measurement by measuring from the middle of the yoke to the shoulder seem and then adding the measurement from the shoulder seem to the cuff. Â I happened to try on the 16 and found the sleeve consistent with what would be labeled a 35 in most off the rack shirts (for example, my Polo Philip 15.5/35 has about the same length sleeve as this, perhaps even 1/4" longer in fact). These shirts are barrel cuffs in a moderate spread collar. Â They are a high quality broadcloth in a white base with light blue and rust pinstripes. Â The pictures make the pattern a little hard to discern because the camera doesn't pick up the blue stripe very well. I'd say that these shirts have a medium fit, though the 18 would be on the slightly slimmer side for someone who wears a 48 suit. Â I measured the 16 as having about a 50 inch chest, and measured the 18 as a 55 chest. Both shirts have about a 5 inch taper through the torso. They also have pretty high armholes. By the way, I should mention that the pattern is really spectacular IMO. It would look especially good with a dark grey or charcoal suit or pants.
