Need advice on whether to get the 38 or 40, but I have no experience with Costume National.
I'm 5'11" 180 and wear a Valentino RED peacoat in a 40, a 38L H&M suit off the rack. The 40 would probably be loose enough to slip an extra layer underneath but hardly billowing, and the difference in the shoulders is negligible. Problem is my extra-long arms tend to make the sleeves on most 38 garments end about a half-inch north of my watch. (For further comparison, I returned a 50/40 Dries van Noten sportcoat that went all the way to the pad of my thumb -- wasn't totally digging the pattern and the working cuffs were going to cost too much to shorten the sleeves).
