A true bespoke shirt is very expensive. (bespoke meaning a unique pattern for you, trial shirt, etc.). Also, shirts suffer more wear and tear from laundry than most other tailored clothing. I'd rather spend money on bespoke jackets/trousers and get a quality MTM shirt. With MTM I get a good fit and most of all my choice of fabrics, collar style and other details. Currently I'm getting my shirts made by Joe at mytailor.com where a good Thomas Mason fabric MTM is around $140-$150. Joe measures you (or you do it online and then they are cut and sewn in Hong Kong). I've also begun using No Man Walks Alone for their "custom" handsewn MTM shirts from Inglese in Italy. They are about twice the price but you get loads of handwork, Italian flair" in the cut, and some beautiful fabric and detail options. I think they are a great value and compare to Kiton shirts I've seen recently at 3-4X their price.
