Question on AE fit/lasts

Usually my biggest problem has to do with finding the correct length - too long and my legs hurt by the end of the day, too short and obviously my toes are smashed to the front of the shoe and are aching. I have tried almost all of the lasts so I sort of know what works and what doesn't, and what sizes I should take.

One thing which is sort of a "wildcard" - Sometimes, regardless of length or anything else, I get a tremendous amount of pain at the joint of my right big toe. This can be excruciating. Trouble is it doesn't show up until a real full day of wear, meaning like 10+ hours. I have had this with my Daltons and more recently with my Jermyns. I noticed that with a lot wear and break-in, the pain will go down but will not disappear completely. In this cold weather it seems to be a lot worse.

Wondering what this is - is this a matter of the last being the wrong shape, the instep, or something else?
