How do you know what countries are included in this?

Set your ship-to country and then start checking out.
They show the total including breakdown of taxes/fees before you have to enter payment.

Crew has an extra 30% off sale items in store and online with code MUSTHAVE.  Some things on sale online are not on sale in store while I've noticed some sale items are cheaper in store than online.  Free shipping online for orders $175 and over.  ALSO, 3% cash back from Ebates if you link J. Crew's site from Ebates.
