Had a couple of shirts made like that - useful in theory, but less so in practice. A link vs. a one-button cuff are at opposite ends of the formaility spectrum, and I never found a need to have a shirt play both roles.
Got four more shirts from Belisario in the mail today. Went with a higher collar this time which I think was a very good idea (I've got a tall neck). I am really pleased with how the collars turned out and I can not wait to wear these shirts.

Yes, I used measurements take from an already existing shirt. You can use body measurements as well, but I've always found those rather unreliable. My previous orders have all been very close to the measurements I entered. My experience so far is that they are consistent from shirt to shirt and are true to the measurements submitted.
