my two favorites for fashion/streetwear are l'Eclairareur on rue des Rosiers in the Fourth and the Martin Margiela store which is an absolute can't miss.

Rue Etienne Marcel for sure, and Rue de Fleurus--instead of APC (which is closed right now anyways, I was just in Paris 3 days ago), roll across the street to Georges de providence--great selection, cool presentation

Lunch in Paris can get very pricey, and I like to eat like the locals, so I always go for a good, less expensive place like Brasserie Balzar (no. 49, rue des Ecoles, 5e - try the poulet roti and tarte au citron), Cremerie Restaurant Polidor ( rue Monsieur le Prince, 6e - blanquette de veau is nice, and the lentil soup is the best), or Perraudin (no. 157, rue St. Jacques, 5e - try the kir Maison). And do yourself a favor: stop at Angelina (226, rue de Rivoli, 1er, just down from the Louvre) and order the chocolat a l'Africaine, which some say is the best in the world. Have a great trip!

Ispahan is the one to have, don't play crazy earth boy! Also, you should know that people will tell you that LaDuree has better Macarons, but they're lying. Jean-Charles Rochoux, a chocolatier, is INCREDIBLE. For a great mid-priced Menu, check out Aux Lyonais. Get the Boudin Noir. For Lunch, I'd check out Fromagerie 31 for some wonderful cheese plates. Not really a full lunch, but quite nice, and delicious. I think there's a place for wonderful ham just up the Street as well (De Rosa or something).

Any more suggestions on good places to eat (cheapish) and shop?
