While I truly like all of the garments that I've bought from my MTM maker over the years, I find the small deviations in fit from one to another a bit troubling. I have specifically instructed that no changes were to be made to my pattern after we dialed it in around the third garment--one of my objectives with MTM was a wardrobe in which every jacket and pair of trousers fit exactly the same--yet I still find small, but noticeable, differences. Am I being too picky? Is this to be expected with the passage of time and changes to the maker's tooling? Or is it just a quality control problem?

Differences in fit are normal and to be expected. Most MTM is cut from blocks so there is no set pattern for you. Consistency has little to do with the pattern as much as controlling the variations in how the cloth changes during tailoring and pressing. Sometimes the more hand work the more variation.

On a related note, how do you feel about the consistency of bespoke (generally speaking)? Would I be able to expect greater consistency with a lower-end bespoke operation such as W.W. Chan, or would the presumably greater amount of handwork complicate this?

Bespoke tailoring is consistent. But it will not be exactly the same everytime it will vary by just a little bit because it is almost all done by hand.

The biggest reason the may not be all the same depends mostly on the fabric. And a little bit on the hand work.
For example I just got a Winter weight suit, that falls better than my regular weight suits. I have one suit that is pretty light weight that fits amazing and is one of my goto suits. But it doesn't fall as well simply because the fabric.
A good bespoke tailor is going to make them very similar to each other. But we are all humans. And hand cutting suits sometimes they are a little off. I had one suit that finished just a little bit big, but it wasn't a problem, he made the suit he can alter it and now it is perfect.
