Vest was made with fabric upside down.

Bingo!

Trouser and vest are cut in the same direction, jacket is reversed, or vice versa. Big booboo for a big brand.

The vest as a whole is cut wrong - the pattern is upside down!

I have to admit that the cutting is too perfect to be true on the sleeves and pockets.
Nice details of the welt pocket on the vest!

But seriously reversed fabric on the waistcoat... hopefully they did not repeat the mistake 2000 times smile.gif

Are the horizontal stripes on a vest and a jacket supposed to line up? Or is it bad to line up? On sweaters, you rarely see the stripes on the arms and body line up.

Low end, entry level clothing may not care if things match. They save cloth and money if they don't concern themselves with pattern matching. At higher quality levels it is expected to match.

Absolutely, but having it matching perfectly on both arms is a stike of luck, 1/8 off is actually consider really good.

I don't think it is a stroke of luck. It is deliberate, done by matching corresponding pattern points marked with notches. Nothing too difficult. If it is off and accepted by a manufacturer it is based on their tolerances. When you give a project to a manufacturer, you give them the tolerances you expect them to perform at. If you say zero tolerance, they have to deliver it as you say.
