Trouble finding shirts that fit. Dress, casual, t-shirts

I have become very disenchanted with shopping for clothes as of late since I can never seem to find shirts that fit me.  It seems like designers must think if you have a long torso you must have a wide one as well.  I like my shirts to fit slim and shaped to my body.  Not necessarily skin tight, but definitely not like I am wearing a tent.  Any recommendations on brands that would fit my measurements well?  I realize I may not get exactly what I want without tailoring or MTM, but thought I would give it a chance to see if people know of OTR brands that may work.

My measurements:

Height: 6'3"

Weight: 190lbs

about a 40" chest

My biggest problems with dress/casual button up shirts is that in order to get a long enough sleeve and torso I have to get a shirt that is much to wide in the body.  With short sleeve shirts, I find that when I find one that fits the width in the body, they are too short and the sleeves are too short.

Shirts can be tapered by a competent seamstress or tailor.

Thanks.  I do realize that in order to get a perfect fit I will most likely need to get shirts tailored, but I was hoping to find some brands that will fit my measurements better than others OTR.

BTW, I noticed your location is PA.  I live in Menlo Park and have been looking for a good tailor.  Any recommendations?

Why waste time and money trying to find OTR shirts that fit when you can just use online MTM and get a perfect fit? Getting shirts that fit should no longer be a problem for anyone:

etc...

Your man in Bob Stankovic, off Oak Grove, Menlo Park

650-323-9614

He's excellent and much recommended by other SF members.
Hint: Unless you stress the urgency of alterations, they tend to be
late. Was the main outside tailor for Wilkes Bashford, Stanford.
