In between sizes - best tailoring approach?

I purchased a J. Crew Mayfair coat but it seems that I fall right between the medium and the large slim. The medium works very well with just a shirt, but is too small with a sweater or sports jacket/suit. The large slim is too big. I'm concerned that if I have a tailor take the large in a little bit so that it fits well with a suit underneath, it'll still be too big to work with just a shirt. Any tips here on prioritizing? I'm in just a shirt 95% of the time but don't want to look awkward when I have more on under the coat.

Avoid small, medium, and large and go for coats with sizes: 36, 38, 40, 42, 44, 46, etc...

Ok then, substitute "40" for medium and "42" for large slim. If have the 42 taken in so it fits well with a suit, will I be able to get away with it if I'm wearin just a shirt most of the time or will it be too big?

You need a larger coat to wear over a suit compared to what you wear over just a shirt. These are different animals. There is no tailoring fix to be had. You need 2 different coats.

You can actually find a jacket for both situations but it will be hard. The most important thing is a good fit in the shoulders (not too tight but good enough to wear with a sweater. The thing that will be in question is the fit of the chest in both scenarios as it can be too tight with a sweater.

Always size up and tailor down, better than fighting puny seam allowances.

You might be able to find an overcoat that is satisfactory for both, but you will never get one coat to fit perfectly both with and without a sportcoat undernearth, it is not possible. It will either be too large without it underneath or too small with it.
