Thrifting Tweed Jackets - Priority for Shoulder or Chest?

Wondering what your thoughts are on this. It's a bit difficult for me as I have a 42 inch chest and 20 inch shoulders and that combination rarely exists especially when buying used. What is more important for a decent fit and in consideration of what my tailor can do? A 46R with 46 inch chest and 20 inch shoulders or a 42R with a 42 chest and 18.5? Seems like I run across that sizing the most.

What do you expect to do with shoulders two inches too small? Insert a patch?

The other way isn't much better. The cost of shrinking the chest plus whatever else you need done will add up.

Avoid buying deals that turn into money pits.

Figured around 19 could be okay (with the shoulders not laying flat) for a thrift purchase.
