Men's/Women's jacket conversion

Occasionally I see a great jacket in my size which just so happens to be a woman's jacket (little structure, with a relatively masculine cut). Is it possible for a tailor to switch the buttons and buttonholes, assuming that the jacket doesn't require radical recutting?

I think it would be helpful if you show a photo of the suit to your tailor. I'm sure most bespoke tailors are able to make alterations regarding your preference.

How exactly do you expect them to move a button-hole?

Recut the lapels if wide enough, or just cut another one on the other side in the manner of double-breasted jackets, I suppose.

Err, what? Recutting the lapels only helps with the lapel button-hole, and even then only if the lapels are very wide to begin with. Are you seriously suggesting cutting back the whole front edge of the coat to remove the button-holes? Do you not realise this will reduce every measurement by like 2 inches and completely mess up the balance of the coat? If the coat is 'in your size' now, then it certainly wouldn't be any more after this operation.

And as for 'in the manner of double-breasted jackets', I don't see how this makes any sense unless you're intending to actually overlap the fronts like a double-breasted coat.

The only way I can possible imagine this working is if you have button-holes on both sides and use a double-button (a short length of thread with a button on each end) to close the coat like a link closure, through both button-holes.

Do you really need a new jacket that bad? You cant find any other mens jackets that fit? It seems like it would be way more work than necessary. It would probably end up costing you more than if you were to just go and buy a new mens jacket.

This and it will noticeably look horrid after all the tailoring and what not.

Just do yourself a favor and look online.  Many people actually have had this problem when it comes to converting especially when you are buying overseas (depending on where you are from).  I personally hate it and even though this "universal size" thing is actually never realized by any of the manufacturers who are making the clothing, at least they are trying to get it somewhat similar so you don't look like an idiot trying to order clothing.  I looked around and found http://www.coatsize.com and that has a few sizing guides, you could use that or just Google it and if not that just use Yahoo or Bing.

Besides swapping the buttons around, wouldn't the OP need to grow some breasts as well to get the proper fit? Maybe plastic surgery or tailoring can take care of this

It sounds to me that there would be a large amount of tailoring work done; almost to the point of completely rebuilding the jacket. Even without "radical" shoulder work, it seems like chest adjustments, waist adjustments, new functional buttons, and dealing with old buttonholes would be required. The tailor can basically make a new suit for you from the cloth, but I doubt it will be worth the cost compared to getting a male suit. I would guess you could have two indochina MTM suits (test case and a follow-up) made for the same price.

I'm sure skilled custom tailors are able to make the necessary alterations. Also, it would be helpful if you can present them a sample or photo of your preferred style.
