Those are perfect measurements (over sized already).
My tops are similar and the shoulders are about .5" wider which is negligible because this is outerwear.
Chest would be perfect with a layered tee or button down
Sleeve is good, 1" longer than say a suit jacket for me but again is negligible as it is outerwear
Size IV would be huge unless the inner knit will account for some of the roominess as well.
So really, how thick is that interior knit of the Alpaca jacket? Enough to go TTS on the jacket? IV = 48 = US38, which I typically am. More like a 38S
IV should be fine. I'm a 44/46 and have a III. Just know that it's not meant to fit tight on the body.

Now I just gotta save up for one of the longer FW coats.
SS is not THAT hot IMO that you'd need to pre-order to buy something you like. There's an increasing number of retailers that carry the line - and many pieces even end up on sale at season end.
