i seem to have the same problem, again and again. in order to show 1/2 inch of shirt cuff, i have my suit sleeves shortened, but the remaining space (a local custom tailor who alters most of my store-bought suits tells me) is not enough to add 4 working buttonholes. what's the deal? my shirt length is 33 inches, surely i am not the only one with this issue.

Why not go with 3 buttonholes?

There is a solution - just make the bottom two functioning and finish the top two buttons with identical but faux buttonhole stitching. This is what most Savile Row tailors do. Plus the sleeve can be lengthened later if you decide to sell the suit.
