No, it is more natural. It is implicit when you meet at a bar that someone will show up first, and that someone will grab a drink so as not to awkwardly stand there doing nothing for 20 minutes in case the other person is late. It is also okay to chat up the bartender or a nearby patron during this time. This also gives you the opportunity to give the bartender your credit card so that there is no awkwardness at check time.

Depends on the weather really. If it's too cold, I'll go straight inside idgaf. But if it's nice and sunny, I'll wait outside. People sitting alone at bars/cafes creep me out.

it depends if you can park the van in wich you are going to drag her near less obvious exit in the back of the palce do that. if not meet her outside with the van parked in front of you.

call her at the last minute and tell her to meat you at a motel instead. if she says no, ride her in the bathroom.

Not my style to have her meat me, but I will meat her at the motel--just not the other way around.
