WalMart(!) carries Starter brand Dri-Start short sleeve tees in white with a barely noticeable logo on the chest that usually doesn't show through most shirts. Buy it in the smallest size that you can so that it will wick moisture away and you will feel dry and comfortable in most situations. They are sold hanging so you can try them on before you buy, unlike packaged tees. Crew neck only, though.

Hanes white v-necks for me as I don't wear a tie on a daily basis. Though I may have to try this grey thing...

Is there a particular reason to wear v-neck as opposed to crew neck? I'm wearing a tie, shouldn't it not matter?

Only reason to wear a v-neck is if you plan on unbuttoning your collar. Then the crew-neck would look tacky. If not, then either will be the same I imagine.

Go vestless. It's actually cooler and you'll sweat less in summer. And it sounds to me that the shirt is too tight.

Unless you never wear a dress shirt without a tie, why not go with a v-neck for more versatility?

Your nipples can shown behind a dress shirt even if it isn't too tight.

I've got some cream/natural V-necks from American Apparel that are pretty good. If you can get that color, it works as well if not better than heather grey at disappearing under a white shirt. White undershirt + white shirt = super-white (very visible).

When I got white linen trousers, it was suggested that I get "nude" color underwear since the color shows through a bit (even white). I'm sure such T-shirts must exist, but I've never seen em.

Hot weather = no undershirt. If a shirt is too revealing, wear a different shirt.

I don't understand the problem with A-shirts and/or white underwear. No one's going to think anything one way or another about seeing the outline or whiter shading -- any more than anyone's going to criticize a woman for her bra being discernable beneath the fabric of her blouse.

Since I became an adult I stopped using an undershirt in the summer (against mom's advice).
Lately I've read this:

"The Role of an Undershirt

For both the short sleeve and long sleeve button up shirts, always wear a 100% cotton undershirt (V neck preferred). Although wearing two layers of clothing sounds warmer than wearing a single layer, a cotton undershirt protects the outer layer from sweat and fabric staining antiperspirants. If you sweat heavily, consider bringing an extra shirt to change into. "

This is very interesting because I live in South East Asia. We don't get cool weather as much and I am not really concerned on my nipple showing as much as ring around the collar.

Thankfully, my nipple is not hairy!
