They are dated, it is actually THE Neapolitan shoulder (dang) and the most difficult to produce, much more than the famed manica camcia which is basically throwing a sleeve into a sleevehead and the more it rumples the happier the clients are compared to that one.
Compared to the fuss that has to be done about the concave shoulder with its special wadding and padding, cloth manipulation and potential canvas modifications also the tailors have a reason to like it not

Difficultly in make + the fact that msot people simply find it odd and garish leads to the fact that most tailors do not offer it (at least actively).
Well made it has of course much less padding than what you see in womens wear and coutire creations and nothing to do with the monstrous pre fabricated pads sometimes used by modern designers.
I will probably let it be with one suit though, it's "special" nonethteless...
