For alterations, try SARTO in Shibuya. I've never been but it's been recommended by others.

I have consistently used Pin Cushion in Nihonbashi. They are on the street behind Takashimaya. They have a contract to do all the alterations for the high-end boutiques in Takashimaya & Mitsukoshi (Hermes, LV, Gucci, etc.). They are good and reasonably priced. Japanese skills required. http://local.yahoo.co.jp/detail/spot...d8a280b982e77/ 東京都中央区日本橋2丁目6-6 03-3281-4580

SARTO in Harajuku and Ginza are good but not cheap at all.

Koda youfuku koubou in Daizawa Setagaya is slightly cheaper

I use B'Reform Works Studio on Meiji Dori near Harajuku - http://www.baycrews.co.jp/reformworks/
I have been very pleased with them - they are thorough and experienced.
