Two people at work do this to me. My reply is always "busy" and I keep moving.

Since I live in a town chock full of old people I often like to give a prostate diagnosis while I'm there. You think they'd be surprised by the attention down there, but more often than not, they're grateful. I like to think of it as doing a public service.
