If you want something like a 1/2" narrower, I'm sure they would be happy to do it. If you want a drastically narrower lapel, they'll probably tell you that it won't quite look right with the rest of the jacket, which is designed to be made with a fairly wide lapel, and what you want is a whole different kind of jacket, which you would be better off ordering from another tailor. They would be right.

I'm not talking 2 inch skinny stuff here. I'm talking moderate width, like 4 inches. That one on medtech is a 5 incher or so

50 bucks says that medtech's is 3.75". It's nowhere close to 5".

Whether a lapel is wide or skinny is determined more by its relationship with the end of the wearer's shoulder, rather than its actual measurement. Where Med_tech's lapel terminates seems pretty right to me - middle of the road, in between the edge of his coat and his shoulder.

But yes, given the width of his tie, you can see that it's no where close to 5"

I wonder if narrow lapels work better with a structured shoulder. Particularly narrow lapels may not look quite right with the Neapolitan shoulder.

You'll have no problem getting a 4 inch lapel from NsM, assuming you're a reasonably proportioned human.

Now that I look at the picture, he may just have a smaller frame than me and I'm thrown off by his shoulder width. I'm about a 41R and he looks to be a 36 or 38

With the exception of early 20th century coats (Blasi vs. Attolini kind of era), most Neapolitan tailors cut a slightly wider lapel to keep with the softer, rounder, widening silhouette.

It's not very wide, but it's just a hair wider than the half way point to the shoulder.

This is nice. Too bad we live in an era where you're considered a doucheface if you dressed that nice when your <50 yrs old. At least in the U.S.
