I am 6'5", 33 waist 44 chest and I find brooks brothers black fleece polos to be a surprisingly good fit. not quite sure if this is what OP is going for though...
and you are neither slim nor wide, really, in the scheme of things.

18.5" shoulders on someone who is 6'5" is not wide. There are guys who are 5'6" who have 18.5" shoulders if they lift. If anything, I'd say you are narrow shouldered for your height/chest/weight. Typically your chest is twice that of your shoulder span, if you are normally built, ex. I have a 35.5" chest and I am about 17.75" exactly from the very edge of my natural shoulder to the other. I have never picked up a weight in my life, I consider myself really skinny but not abnormally built. If I ate a lot more and lifted I'd probably be 36-37" chest/18" s2s.

While you are tall and slender, I wouldn't think you would have any problems from having "broad shoulders" based on the measurements you posted and your pictures. There should be plenty of polo brands that fit you nicely without being too tight. Your only real concern should be finding ones long enough for your body, while having a good slender, not skinny, fit. If you have trouble in this area, try brands that carry Tall sizes like JCrew, Polo, BR and Gap.

What about all of the above?

I would start in this order:

Go to J Crew. Do their mediums fit you fine? These will be the cheapest. If not, Go to Ralph Lauren. These will be more expensive. Do their custom fit (remember CUSTOM FIT, not REGULAR FIT) size large fit you fine? These are more expensive, but are well made.

BR might be your best bet, but you can only get the medium Talls online. You're not going to be able to try them on, so you'll just have to go with me on blind faith for this one.

Thanks for the specific guide! Now I just need to figure out where in podunk Cedar Rapids, Iowa I can find these specific brands/styles. I'll give it a try, and if it comes down to it, maybe pick up one or two of the Tall sizes at BR online for $40 a pop. Worth it if it'll last.

Don't ever pay full price for Banana. There's always a 30% coupon floating around. JCrew has a final sale going on right now - you might be able to pick up a medium really cheap since you're in bumblefuck (no offense). If $40 is steep, I would pass on the Ralph Lauren custom fit.

OP, try All Saints, I have a polo from there. Don't get a slim fit one as those are skin tight, but regular ones are pretty slim in the arms. Quality is decent. Don't pay for price.

Heh, what does that even mean?

I should ask - what are the key measurements that need to be correct on any given shirt that a tailor can't fix, so I make sure I get that right on my purchases?

Yeah, pretty much all these suggestions. It's a great place to start. If all of those don't work, then report back and we'll try to think of something else to help ya out.

This post (my first post on this forum) isn't much of a contribution since I can offer no help, but I find it funny that almost every guy on the face of the planet thinks (or wants to think) that they have broad shoulders. I was in fact one of these people, but I realized I was merely experiencing delusions of grandeur because I have quite narrow shoulders which sucks to say as a guy, but hey, it's true. So I guess my contribution is saying that no you don't have anywhere near broad shoulders OP--average at best, perhaps a bit on the narrow side. So despite me being nowhere near your size, it's probably something other than your shoulders that's giving you trouble in finding a good fit. What brand/fit that would be best for you, I have no clue. I have major fitting issues too, mainly due to me being on the opposite end of the spectrum--too small. Best of luck.
