Where do you get a form like this?

You can find a half mannequin for like $40. I don't know why you would pay $800.

Since that's my photo, I guess I should answer. I picked that up on eBay for a total of about £25 or so inc. delivery.

Try search terms inc. mannequin, tailor's dummy, tailor's form, etc, etc. Should be able to snag one second hand in good nick (the one I got was being sold by an ex fashion student I think).
