You broke up for a reason. Say nothing, or at most exchange pleasantries. Any less and she'll think you're a wimp.

Everyone is telling you to give it up but that's not what you asked for. I wish people would read the OPs more carefully.
First, make sure an appropriate song "happens" to come on in the background. I would go for this:
Let it get going into the lyrics a good minute or two before you get to this next bit.
Sneak in a reference to some cheeky moment that you once shared, something that will make her laugh. Break that ice, then segue into how you heard the song the other day and that it frightened you. It frightened you that you two could become strangers and that that could be OK. You don't want to become strangers with her, you couldn't stand for that to be OK.
Gaze off into the distance. Tell her you feel fucked up without her, like an empty blue-gray sky after a burning sunset, that you are just cold twilight, no sun, no moon, no stars. You would give up happiness forever just to run your fingers across her face and into her hair just once more, to brush your lips so softly against hers and to come back and press them harder. Look back at her and see if she's let her guard down. Just look for that one split second, when her eyes lose focus, that is your chance.

Firstly, when a woman has moved on, it means you have dropped below her "desirable" level. If you continue to pester her, she will think you a wimp and you drop further points.
What you need to do is stop contacting her for a while and maybe decide that you too need to move on. Realize that getting her back is not gonna change all the bad feelings you have driving her around.

How should you handle it? Like a gentleman. Be charming, be nice, try and move past the anger and just be friendly towards one another.
What can you say to get her back? Nothing we can tell you. Just be honest with each other. I would advise against trying to get back together in that moment. It's been 3 months and that's a lot of time, try and view things from a new perspective and really try to determine if you even want to be back with her. If it was meant to be, it was meant to be. There's nothing you can do to force the situation.

This is not true, not fresh after a breakup. She might just be curious to see what it's like without you, but still harbors feelings for you. Do not listen to them, OP, follow your heart. Now or never, carpe diem!
