I've never personally used one but I have had clients use them in certain deals when there's a lot of money involved.

As an example, one client paid a few hundred thousand for a particular collectible automobile from overseas. The money was held in an escrow account at a reputable bank until the car was delivered and inspected, then the money was released.

If the item involved costs more than a few thousand dollars use something other than paypal.

I know a guy who bought a container load of pinball machines from south american.
It was several thousand dollars. He used an escrow service.
The whole thing sounded like an incredible PITA but it all turned out well.
I can't remember if he bought it via ebay or if he just got in contact with the guy via ebay.

I just pay after services are rendered. Although, you do run into the issue of sub par service from time to time... I imagine withholding payment could have some repercussions.
