Here's a Semi-bespoke dress shoes made by SAION for an extra wide and short feet. Not sure if you need those extra tall heels (perhaps the owner wanted them), but overall it manages to look quite sharp with a tight waist
I'm in Aus so I'm pretty closeI don't think we have many access to the superb Japanese makers here in the US.

lololol, a local shoe repair shop taught me that a pitched heel will make the 'extra' height look less obvious

Maybe not, I am 5' 6" so not extremely short but still like to wear shoes or boots with a visibly higher heel. I have never and will never wear elevator pads because high heel cups look so bad. But a nicely curved cuban or cavalry heel can be really nice. I am starting to working on a boot with a 1 5/8" cavalry heel these days
We are the same height, and if one's not gonna reach 5' 9' (around average height), I wonder why do it at all.
TBH I am OK with my height as my build is relatively proportional but I really like the look of heels. I went to mexico this summer and saw some guys wearing cuban heels and thought it really looked good. I also like the look of alot of the old shoes you would see in early 20th century fashion plates and there was this one Saion bespoke shoe that had this cuban like heel I liked. I also grew up wearing cowboy boots and really miss those heels.
