Help with shortening Shirt Sleeves

I've never really read a single good post about what a tailor can and can't do concerning shortening a shirt sleeve. Assume the shirt in question has gauntlet buttons.

Just how far can they take up a sleeve without messing up the position of the gauntlet button? Can the gauntlet button and buttonhole be completely relocated? (If yes, what does this normally cost?)

I'm looking specifically for information about what a tailor can do to an OTR at a reasonable cost. I know most of you will reply "Go MTM", but getting an answer like that isnt the purpose of this question.

Every once in a while, I come across killer deals with OTR shirts that look really good but sizing is usually a pain. Sleeve length wise, I have short arms and often fit into a size 38 but my neck is size 39. Most size 39 sleeves seem about 1-2 inches too long for me.

On a high quality shirt, my understanding was that the gauntlet piece could be removed. So if you remove the cuff and the gauntlet, you just have to cut the sleeve shorter, retaper/enlarge the gauntlet area if necessary, and reattach the gauntlet and cuff. If the shortening is less than an inch, it may not be a big deal to move the gauntlet.

that's how my tailor said he would shorten the sleeve.

i've heard of stories of shortening at the shoulder, however.
