Its terrible here in the corn belt. ~6 women passed by the window while I was having my haircut yesterday -- only 1 appeared to be modestly fit. I believe it to be a function of limited opportunities for recreation and the need to drive nearly everywhere. Chicago is an oasis of fitness by comparison.
I have to say, most of the heavy people I see or know in NYC tend to be older at 40+ and/or from out of town. Most people I know that live in NYC are actually pretty darn thin or in good shape.
