The Prodigy at the Hove Festival in 2009. Slight ringing the next day, went away less than 36 hours after the concert ended.

Lots of teenage punk concerts put on by people who didn't know how to run sound. Two years in a row I had MBV reunion tickets, had pneumonia both times. Fuuuuuuuu...

Worst sound I've heard at a 'real' concert was Clapton's farewell tour in 2000 or 2001. His sound wasn't bad, but the opener (Doyle Bramhall III, I think?) was cranked over the PA, very sharp/trebley and hurt to listen to.
