I say doc (like doc-tor) when speaking English, and doc with an Italian accent when speaking Italian. I don't really know how to explain the difference, but why is pretty much right on. Spelling out the letters sounds weird in English, weirder even in Italian. I've never heard it done, and have heard the expression countless times. Also, Italians always call my dog Owen Howen. Worse, they turn it into Howie. I would be happier if they just called him D.O.C.
