Thanks, it's one of my favorites. Alas, I can't be too specific, but I can send you in the right direction. All of my shirts are MTM by Joe Hemrajani (MyTailor.com). It was one of the first shirts I ordered, and I don't recall which book the cloth came out of. I wouldn't be surprised if it were still available.
I've been quite happy with all of the shirts they've made for me, and have referred friends as well. Will from A Suitable Wardrobe also has his shirts made by Joe.
