The tendons in my elbow hurt like hell from squats even though I switched to the grip suggested in Starting Strength two weeks ago when they first started to hurt.
Any suggestions other than ice and NSAIDs?

I had the same problem, turned out I had the bar too low on my back and basically had all the weight on my elbows, putting it higher on my back helped a lot as did elbows sleeves. If the pain persists you could consider wrapping a pair of straps around the bar and holding onto them in front of you as handles to take al the strain off your shoulder and arms.
