If you're doing presses properly then you don't really need anything beyond that and maybe shrugs for your traps. That is, pushing your body forward as soon as the bar goes over your head and squeezing your traps at the end of the lift.

honestly man, anytime you're talking about lifting there's almost NEVER just one answer. the answer depends on so much. one thing that others failed to ask about that is majorly important is your goals. want to get bigger? stronger? leaner? there are almost no lifts that, when performed properly, are useless.
im a huge proponent of variety. if you perform any one movement for too long you'll eventually run into repetative motion problems. even dorian yates, one of body buildings greats, had problems with this. if you're just doing them with really light weight as part of a circuit you wont get huge traps. a few others had decent suggestions for semi-substitutes, but just dont get in the groove of doing only isolation movements. i truly believe that compound lifts are the key to most all good physiques.
as for the pain, that could be caused by any number of issues and i'm not even gonna try to guess over the internet, but if it hurts dont do it

FWIW, I did upright rows for years with no ill effects that I could discern.

In fact, I have been doing them again as part of my workout for the past year or so, again with no discernible ill-effects on my 67-year-old body (although I'm using only about 70% of the weight I did when I was in my prime).
