I'm looking to get from penn station(after getting in from NJ) to montefiore medical center in the Bronx. What would be the best way to get from penn to Montefiore?

Walk one block east to the 34th Street/Herald Sq station and take a Bronx-bound D train to the last stop (205th Street).

There's a great site for this also available via mobile phone - hopstop.com

There, you can put in anyplace that you are in NY and any destination and it will tell you how to get there via the subway. Saved my ass more times than I can count.
