I'd suggest a crisp white shirt as it makes the suit stand out more overall...I think you answered mostly all your questions.

When I do wear a suit especially for a formal occasion I don't wear a watch. But to each his own..

I don't have the belt loops on my suit trousers but  I can't live without it on  my casual pants.
